We can all say education is a big requisite in life, but do we really spend the appropriate time to it? . We sure not†¦, at least I didn’t for a couple of years. Freshman year on high school could be hat year you would never forget. Becomes a new life style, met new people, new hobbies, and new interests. Basically turns out to your biggest change in life. My high school education could of say it was more fun and adventurous than those kids who actually are focus to graduate with a high GPA, get an efficient scholarship, and attend to a reputable university. I started out my freshman year good. I was attending to class, participating and doing class work, I was doing homework. I had no problems, then after my freshman year was over I had many friends that I was expected. Then I got to meet my true friends which I still hang out and do crazy stuff. My sophomore and junior year I went out of control laterally. I never was in school, just to go get my free lunch because, who didn’t liked to be in lunch? Right. To be clearer, anywhere was a better place than school. Yeah I had so much fun, memories than I would never forget, unexpected trips, water balloons fights, ride horsing, and four wheeling, ditching parties, even run away from cops. But like how it needed to happened, it happened. My slacking in grades would be known eventually by my mom. There’s one thing that I’ll be always glad for, I have the most overprotective mom in the world. Her reaction was immediately that the next day she knew about I was not going to school, she was there talking to the principle asking him to make a stop of it. All the trouble that I was into, and all the talks my mom and I had, it made my mind expand to reality and to convince myself that I was not doing good in school and that would affect my future. I catch up my senior year picking up my GPA and grades to proudly get thehigh school education and be able to register at CNM. College is a different level than high school by too much. Here you cannot take the risk to slack not even a little. If you slack not showing up, or not turning homework, grades are really affected. College has help me to raise myself as an adult and take proprieties first. That you are who build your future, that hard work always pays off. I’m thankful to attend to college.

Leaders with a passionate inspiration will motivate their people by including them in the creation of the corporate vision. Empowered employees require a leader willing to devote time, effort, and energy to establish a core of shared values. Leaders that invest themselves, their resources as well as time are the foundation for building an emotionally intelligent organization. Promotion of emotional intelligence (EQ) requires a broad scope of soft-skills that leaders can use for the benefit of their organization. Foundations of Emotional Intelligence Emotional leadership is the spark that ignites a companys performance, creating a bonfire of success or a landscape of ashes (Goleman, Boyatzis Mckee, 2001). Goleman (1996) defines emotional intelligence as the capacity for recognizing ones feelings and those of others, for motivating ourselves, and for managing emotions well in relationship with self and others. His framework includes four major branches that have grown from the seed of EQ: self-awareness, self-management, social awareness, and relationship management. Kreitner Kinicki (2010) affirm the first two are personal competencies and determines how we handle ourselves; the second two are social competencies and determine how we handle our relationships with others (p. 145). Daft (2005) defines emotional intelligence as the ability to recognize, comprehend and assimilate emotions in self and others and stresses that everyone has the ability to increase their EQ (p. 145). According to Daft, (2005) Managing emotions does not mean suppressing or denying them but understanding them and using that understanding to deal with situations productively (p. 146). Benefits of Emotional Intelligence Skills It is easy to understand the importance of emotional intelligence in regards to managing stress, increasing motivation, and productive conflict resolution, as these have been linked to improved overall organizational effectiveness (Daft, 2005). Individuals and organizations that can learn to operate in emotionally intelligent ways will remain vital and dynamic in todays competitive marketplace. Leaders with high levels of EQ are able to instill unity and create high performance teams that are cohesive in their work efforts. In todays fast paced, static environment, organizations that introduce and implement EQ are able to create sustainable changes in an established corporate culture and build a new culture of shared responsibility with a united purpose (Daft, 2005). Emotional intelligence is exactly what the name implies. The basis of an emotionally competent leader is one with an inspiring vision for change with increased adaptability. The emotional stability of a leader can affect the whole organization by being a positive role model that is able to motivate and inspires others by their empathetic abilities (Daft, 2005). Golemans research, over several years with diverse types of organizations has shown the ability to predict high performance individuals and teams through soft skill competencies associated with EQ. Organizational training and development of EQ for leaders and managers have been shown to increase employee productivity, reduce apathy, and increase employee fulfillment, and job stability. The Process of Organizational Emotional Intelligence Emotional Intelligence is not just managing emotions: it involves engaging emotional intelligence insights and tools combined with the potential of the current rational tools. The process for learning EQ follows a logical progression. First the organizations top leaders should be comfortable with using EQ competencies, such as self-awareness to build empathy, coach others, resolve conflict, and improve their political awareness and teamwork. Organizational leaders need a clear definition of the critical importance of EQ through conducting a detailed self-assessment of EQ competency skills for all staff members. This is not something that can be accomplished in a one-week seminar. It requires repeated application of basic competencies of EQ at home, work or social gatherings. This requires insight into leadership capacity and increasing organizational performance by benchmarking and strategically leveraging diversity (Druskat Wolff, 2001). Leaders and managers provide coaching or men toring support and should establish a 360-degree feedback for visible improvements, as well as a high return of the organizations initial investment that can improve corporate culture for a lifetime. It has been determined that consistent utilization of emotional intelligence encourages staff members towards intentional communication, in terms of people issues and individual response to change (Fariselli Ghini, 2005). Extensive research by Goleman, Boyatzis Mckee (2001) have shown that high levels of emotional intelligence, create organizational cultures where learning, trust, sharing of information, and healthy risk-taking increases on a consistent basis. Are You A Man or A Monkey ? A Disturbing Movement of Anti-Intellectualism in America "It’s more valuable to see with the eye in one’s heart, rather than see with the eye in one’s head." The epic crusade of science and technology versus theology, both religions of sorts dating back in time more years than any of us can begin to comprehend. Maybe that is why, as a whole, we have such a difficult time discerning between the two, or rather, why we fail to see clearly the true meaning that lies behind the propaganda of either. The arguments on either side are significant and carry as much reason and weight as the other. Thus, we simply cannot refuse to make a judgment before looking critically into the logistics surrounding the propaganda of each theory. God’s diplomats, the Bible-thumping, prophesizing blow-hards much like Brady in Inherit the Wind, are as much the bigoted and biased, sacrilegious and amoral attention-seekers as they proclaim the evolutionists to be. However, their chosen doctrine cannot be overlooked, as I myself am deeply devoted to it’s teachings. Brady and others like him fight from the backbone of Faith. I don’t believe in the literal deciphering of the Bible, but that it is a book of ideals that we must trust in it’s veracity. It isn’t meant to be explained! Ironically, the thing that people are the most hungry for, meaning, is the one thing that science hasn’t been able to give them. Enter God, the means that mankind has clung to for purpose. If there isn’t a God, does that mean that 95% of the world is suffering from some sort of mass dillusion? There may be a thousand arguments against there being a supreme being that we can think of, but it’s all those reasons that we cannot think of that allow him to continue to exist as a necessity in our hearts and minds. True, in the past Galileo, Copernicus and others have proven that the Church can be wrong -- and I agree. Yet the Church, like humanity, has the right to make a mistake and reassess their beliefs. It doesn’t mean all they say is false, not at all! I couldn’t imagine living in a world where God didn’t exist -- I wouldn’t want to. Turn around 360 degrees and you are back facing the same direction, now science lies in front of you where religion so recently resided. Politics, science, philosophy, theology, technology -- it’s so easy to become confused. Science is a truth, no matter how adamantly we decree it otherwise.

The Battle of Formats There are two new formats of DVD's coming out one is Blu-ray and one is HD-DVD, will this become the battle of VHS vs. BetaMax and which will be in your home. First, let me give you some history on both of these new forms of DVD's. Blue-ray which was invented by a group of companies called Blue-ray Disc Association. It consists of companies like Sony, Apple, Dell, HP, and many more. Blue-ray will hold 25 gigabytes and will be the same size and shape of a DVD. The only difference is that it uses a different laser which is the color blue. Now you know why they called it this. The big factor that will push Blue-ray to be in your home is that Paramount Pictures, Universal Studios, and Warner Bros will be big supporters of Blue-ray. Another big feature is that it will feature DRM which is digital rights management. Blue-ray has another form of encryption called BD+ that will allow the companies to change the encryption. This will make it very hard to make copies of their products like movies and software. HD-DVD was introduced on the DVD forum and on November 19, 2003 they decided that the HD-DVD format be the new DVD to output HD movies. HD DVD was originally called Advanced Optical Disc. Like Blue-ray it is the same size as Cd's and DVD's. HD-DVD's will only hold 15 gigabytes, 10 less than Blue-ray. HD-DVD uses a red laser to burn information and read information. In the article â€Å"When Rapists Walk Free,† Mukhtaran Bibi received a raping because of a village dispute. Because Bibi lives in Pakistan, Nicholas Kristoff, the article’s author, was led to believe that Hatun’s village was Muslim although in the second sentence of the article, He clearly writes that she lives in the Punjab area of Pakistan. The Punjab region hosts a number of religions aside from Islam. So, by reading the sentence that Bibi received, I clearly know that her village is not Muslim, because nowhere in Islam it says to gang rape a girl because of a village dispute. â€Å"The New Berlin Wall,† by Peter Schneider, does the same mistake in confusing another religion with Islam. Hatun Surucu was murdered by her 3 brothers because she started to live like a German. Because this happened in Turkey, the author assumed they were Muslim. Although in the first sentence of the second paragraph, Schneider informed the reader that Surucu’s family was Kurdish. Kurdism is a far branch of Islam that practices many different actions than Muslims and is not to be confused with Islam. So, in response to the prompt, I believe that no, the German government should not interfere with the status of women in their Muslim communities, but they should act out and address the status of women in Germany’s Kurdish communities. The reasons why I believe Germany should address the status of Kurdish women is because the German Kurds are now living in a different nation than their own and if they cant adjust, they should get penalized for it. Also, the government should push nationwide freedom and if Germany doesn’t feel that the Kurdish women in their country are getting the liberty they deserve, it is their duty as a nation to change that. When an immigrant comes into a country and decides to keep his past culture with him, fighting and deaths are the end result as shown with the Hatun Surucu case. Her brothers kept their Kurdish practices of honor deaths and ended up killing their sister. An example ritual is â€Å"Generally choosing the youngest to carry out such murders.† If the Surucu family adapted their Kurdish traditions and accepted Hatun as becoming German, she might not have been brutally murdered. Also, the reason the Surucus immigrated was to find a better life.

Learning institutions in America before public education

In the early part of American history, education was not for everyone. The masses usually came from agricultural communities were planting and harvesting crops were more important than education. On the other side of the fence, were the wealthy families, the businessmen, the landed and the politicians who were able to enjoy the benefits of education. The kind of education that the poor got was a few weeks of schooling during winter where teachers were not trained and school houses were decrepit and materials were lacking. The lessons were limited to basic skills to enable the children to write their names and count hens. The public education system that we presently have is a far cry from what has been in the past century. We owe the present form of education we have to Horace Mann, who campaigned for and worked to bring about social reforms and give each American the right to free education. Mann through his enthusiasm and devotion secured within his own state public support for schools. He encouraged businessmen to support building public schools since he believed that educating the masses would lead to greater economic progress. It goes to say that having a literate nation would spell more skilled and competent workers who would be the backbone of the factories at that time. More so, when the masses were given the right to vote, a growing awareness of the government’s obligations to its people came to light, the clamor for free education was strongest at this period. Mann faced a few difficulties in the form of opposition from prominent politicians and wealthy businessman and the clergy for education would be an equalizing factor between the rich and the poor and most especially because public schools would be competing with private and sectarian schools. Horace Mann also advocated public education for the masses as a means of bringing equality to the society. He argued that pubic schools made it possible for all citizens to exercise their right to free education without the encumbrances of religion or financial burdens. The individual could go to school without having to believe in something that is opposed to his cultural background and everyone could at least finish high school without the need to apply for loans or maintain a scholarship. The poor could concentrate on learning and in some ways to prepare for their future. Furthermore, Mann was able to integrate the schools in his state to a system that became the basis for the public education system in the country. He also advocated the establishment of normal schools to train teachers; he was against corporal punishment and instead focused and emphasized better teaching methods. Thus education gained the attention and funding that it never before had, and was established as one of the institutions of society. By the latter part of Mann’s life, the number of elementary and secondary schools had increased dramatically all over the country, but he did not stop there. He went on to work on changing higher educational system wherein the poor had the opportunity to be admitted to colleges until his death. Thus the door to educational advancement and success was opened to the masses. Â   Â

Paranormal Existence Is someone or something else among us at this moment, in this room, something we can’t even see? Have you ever wondered what else is really out there? Unexplained happenings occur every day. Spirits or ghosts are thought of just being people that have passed away but there has to be more to it. Paranormal activity is defined as experiences that lie outside the range of normal experience or scientific explanation.At some point in everyone’s life they will experience some kind of unexplained activity. Many people ask the question if it actually exists or if it’s all in their head. Scientists often say that these so called â€Å"spirits† do not exist. Only personal experiences can interpret your own feelings about paranormal existences. My story is about a spirit that lived on past their death. One night a couple months ago me and my friends were playing on her wigi board. Her dad had died 5 years earlier due to a horrific car crash.Her f amily had always seen balls roll across the room and other strange things happen without explanation; they always assumed it was him. When we started playing around with the board the first letter of the girl’s dads name showed up and her eyes instantly filled up with tears, like a helpless child left all alone, the second letter was the second letter to his name, the same with the third. By this time my friend was balling, she of course knew it was her father. We continued playing until the last letter of his name showed up.At this moment we heard a vase upstairs shatter into a million pieces, just like the car windshield had done when his helpless body had gone through it, instantly killed. We ran upstairs to find the vase and its contents of flowers spread all over the floor. Then we replaced it with a new vase and reassembled the flowers to how they were. The next morning her mother walked downstairs with a flower in her hand and asked, â€Å"Why did you put a flower nex t to my bed? † That night changed the way I thought about spirits forever.I never thought anything like that could ever happen to me. Anyone who experienced that kind of activity and the intensity in that room could easily say they felt a presence surrounding them. In the moment of that glass shattering was the one of the scariest moments of my life. I will always remember the terror racing through me, my heart pounding, sounding of horse’s strong hooves beating against the track, like thunder rolling in and the suspense leading up to the storm.Saying spirits like this don’t exist is outrageous considering how many people actually experience real activity. Some people don’t want to believe this is real simply because they are scared of it. Many others find it very interesting to study about and explore the possibilities of what else could potentially be out there with us in our everyday lives and something you aren’t even able to see. Everyone has d ifferent views on if paranormal existences are actually real. Mysterious things could happen at any time, and when they do, be prepared.

Zoe’s Tale PART I Chapter Two

I climbed out onto the roof through my bedroom window and looked back at Hickory. â€Å"Hand me those binoculars,† I said. It did – and then climbed out the window with me. Since you've probably never seen it I'll have you know it's a pretty impressive sight to watch an Obin unfold itself to get through a window. Very graceful, with no real analogue to any human movement you might want to describe. The universe, it has aliens in it. And they are. (Obin: â€Å"it,† not â€Å"he† or â€Å"she.† Because they're hermaphrodites. That means male and female sex organs. Go ahead and have your giggle. I'll wait. Okay, done? Good.) Hickory was on the roof with me; Dickory was outside the house, more or less spotting me in case I should trip or feel suddenly despondent, and then fall or leap off the roof. This is their standard practice when I climb out my window: one with me, one on the ground. And they're obvious about it; when I was a little kid Mom or Dad would see Dickory blow out the door and hang around just below the roof, and then yell up the stairs for me to get back into my room. Having paranoid alien pals has a downside. For the record: I've never fallen off the roof. Well, once. When I was ten. But there were extenuating circumstances. That doesn't count. Anyway, I didn't have to worry about either John or Jane telling me to get back into the house this time. They stopped doing that when I became a teenager. Besides, they were the reason I was up on the roof in the first place. â€Å"There they are,† I said, and pointed for Hickory's benefit. Mom and Dad and my green friend were standing in the middle of our sorghum field, a few hundred meters out. I raised my binoculars and they went from being hash marks to being actual people. Green man had his back to me, but he was saying something, because both Jane and John were looking at him intently. There was a rustle at Jane's feet, and then Babar popped up his head. Mom reached down to scratch him. â€Å"I wonder what he's talking to them about,† I said. â€Å"They're too far away,† Hickory said. I turned to it to make a comment along the lines of no kidding, genius. Then I saw the consciousness collar around its neck and was reminded that in addition to providing Hickory and Dickory with sentience – with their idea of who they were – their collars also gave them expanded senses, which were mostly devoted to keeping me out of trouble. I was also reminded that their consciousness collars were why they were here in the first place. My father – my biological father – created them for the Obin. I was also reminded that they were why I was here, too. Still here, I mean. Alive. But I didn't go down that road of thought. â€Å"I thought those things were useful,† I said, pointing to the collar. Hickory lightly touched the collar. â€Å"The collars do many things,† it said. â€Å"Enabling us to hear a conversation hundreds of meters away, and in the middle of a grain field, is not one of them.† â€Å"So you're useless,† I said. Hickory nodded its head. â€Å"As you say,† it said, in its noncommittal way. â€Å"It's no fun mocking you,† I said. â€Å"I'm sorry,† Hickory said. And the thing of it was, Hickory really was sorry. It's not easy being a funny, sarcastic thing when most of who you were depended on a machine you wore around your neck. Generating one's own prosthetic identity takes more concentration than you might expect. Managing a well-balanced sense of sarcasm above and beyond that is a little much to ask for. I reached over and gave Hickory a hug. It was a funny thing. Hickory and Dickory were here for me; to know me, to learn from me, to protect me, and if need be to die for me. And here I was, feeling protective of them, and feeling a little sad for them, too. My father – my biological father – gave them consciousness, something the Obin had lacked and had been searching for, for the entire history of their species. But he didn't make consciousness easy for them. Hickory accepted my hug and tentatively touched my head; it can be shy when I'm suddenly demonstrative. I took care not to lay it on too thick with the Obin. If I get too emotional it can mess up their consciousness. They're sensitive to when I get overwrought. So I backed up and then looked toward my parents again with the binoculars. Now John was saying something, with one of his patented half-cocked smiles. His smile erased when our visitor started talking again. â€Å"I wonder who he is,† I said. â€Å"He is General Samuel Rybicki,† Hickory said. This got another glance back from me. â€Å"How do you know that?† I said. â€Å"It is our business to know about who visits you and your family,† Hickory said, and touched its collar again. â€Å"We queried him the moment he landed. Information about him is in our database. He is a liaison between your Civil Defense Forces and your Department of Colonization. He coordinates the protection of your new colonies.† â€Å"Huckleberry isn't a new colony,† I said. It wasn't; it had been colonized for fifty or sixty years by the time we arrived. More than enough time to flatten out all the scary bumps new colonies face, and for the human population to become too big for invaders to scrape off the planet. Hopefully. â€Å"What do you think he wants from my parents?† I asked. â€Å"We don't know,† Hickory said. â€Å"He didn't say anything to you while he was waiting for John and Jane to show up?† I said. â€Å"No,† Hickory said. â€Å"He kept to himself.† â€Å"Well, sure,† I said. â€Å"Probably because you scared the crap out of him.† â€Å"He left no feces,† Hickory said. I snorted. â€Å"I sometimes question your alleged lack of humor,† I said. â€Å"I meant he was too intimidated by you to say anything.† â€Å"We assumed that was why you had us stay with him,† Hickory said. â€Å"Well, yeah,† I said. â€Å"But if I knew he was a general, maybe I wouldn't have given him such a hard time.† I pointed to my parents. â€Å"I don't want them getting any grief because I thought it would be fun to mess with this guy's head.† â€Å"I think someone of his rank would not come all this way to be deterred by you,† Hickory said. A list of snappy retorts popped in my head, begging to be used. I ignored them all. â€Å"You think he's here on some serious mission?† I asked. â€Å"He is a general,† Hickory said. â€Å"And he is here.† I looked back through the binoculars again. General Rybicki – as I now knew him – had turned just a bit, and I could see his face a little more clearly. He was talking to Jane, but then turned a bit to say something to Dad. I lingered on Mom for a minute. Her face was locked up tight; whatever was going on, she wasn't very happy about it. Mom turned her head a bit and suddenly she was looking directly at me, like she knew I was watching her. â€Å"How does she do that?† I said. When Jane was Special Forces, she had a body that was even more genetically modified than the ones regular soldiers got. But like Dad, when she left the service, she got put into a normal human body. She's not superhuman anymore. She's just scary observant. Which is close to the same thing. I didn't get away with much of anything growing up. Her attention turned back to General Rybicki, who was addressing her again. I looked up at Hickory. â€Å"What I want to know is why they're talking in the sorghum field,† I said. â€Å"General Rybicki asked your parents if there was someplace they could speak in private,† Hickory said. â€Å"He indicated in particular that he wanted to speak away from Dickory and me.† â€Å"Were you recording when you were with him?† I asked. Hickory and Dickory had recording devices in their collars that recorded sounds, images and emotional data. Those recordings were sent back to other Obin, so they could experience what it's like to have quality time with me. Odd? Yes. Intrusive? Sometimes, but not usually. Unless I start thinking about it, and then I focus on the fact that, why yes, an entire alien race got to experience my puberty through the eyes of Hickory and Dickory. There's nothing like sharing menarche with a billion hermaphrodites. I think it was everyone's first time. â€Å"We were not recording with him,† Hickory said. â€Å"Okay, good,† I said. â€Å"I'm recording now,† Hickory said. â€Å"Oh. Well, I'm not sure you should be,† I said, waving out toward my parents. â€Å"I don't want them getting in trouble.† â€Å"This is allowed under our treaty with your government,† Hickory said. â€Å"We're allowed to record all you allow us to record, and to report everything that we experience. My government knew that General Rybicki had visited the moment Dickory and I sent our data query. If General Rybicki wanted his visit to remain secret, he should have met your parents elsewhere.† I chose not to dwell on the fact that significant portions of my life were subject to treaty negotiation. â€Å"I don't think he knew you were here,† I said. â€Å"He seemed surprised when I sicced you on him.† â€Å"His ignorance of us or of the Obin treaty with the Colonial Union is not our problem,† Hickory said. â€Å"I guess not,† I said, a little out of sorts. â€Å"Would you like me to stop recording?† Hickory asked. I could hear the tremble on the edge of its voice. If I wasn't careful about how I showed my annoyance I could send Hickory into an emotional cascade. Then it'd have what amounted to a temporary nervous breakdown right there on the roof. That'd be no good. He could fall off and snap his snaky little neck. â€Å"It's fine,† I said, and I tried to sound more conciliatory than I really felt. â€Å"It's too late now anyway.† Hickory visibly relaxed; I held in a sigh and gazed down at my shoes. â€Å"They're coming back to the house,† Hickory said, and motioned toward my parents. I followed its hand; my parents and General Rybicki were indeed heading back our way. I thought about going back into the house but then I saw Mom look directly at me, again. Yup, she'd seen me earlier. The chances were pretty good she knew we had been up there all that time. Dad didn't look up the entire walk back. He was already lost in thought. When that happened it was like the world collapsing in around him; he didn't see anything else until he was done dealing with what he was dealing with. I suspected I wouldn't see much of him tonight. As they cleared the sorghum field, General Rybicki stopped and shook Dad's hand; Mom kept herself out of handshaking distance. Then he headed back toward his floater. Babar, who had followed the three of them into the field, broke off toward the general to get in one last petting. He got it after the general got to the floater, then padded back to the house. The floater opened its door to let the general in. The general stopped, looked directly at me, and waved. This sport taught me the importance of concentrating on what we do. It also taught me prudence and helped me to achieve a calm mind.I decided to interact more with the world that I live in. Hence, I started to study the various changes taking place in the world. In this study, I came to realize that the multinational corporations had brought about the process of globalization. College success very closely relates to personal responsibility in that, in order to be successful in college you have to take personal responsibility to complete your assignments and courses on time in order to graduate when you are scheduled to. To be specific, having personal responsibility means you have to be accountable for your actions. When people are holding themselves accountable for their actions they tend to work harder to complete the task correctly. They also will be certain to turn it in when it is due. Another part of the equation is taking responsibility for yourself. Being responsible means you do your assignments on time. It also means you did them correctly. By being responsible you show others you can be depended upon to fulfill your part of the task. The final portion of my definition is feeling a sense of accomplishment. Finishing a task on time and correctly gives a person a great sense of accomplishment. Being able to contribute to the team goals and be acknowledged for it makes a person feel good. I know I take a sense of pride when I am able to accomplish something I’ve been tasked with and finish it to the best of my ability and the rest of the team is happy with my efforts. In conclusion, although some people rather let others handle things, people need to be accountable for their actions for three main reasons. Pancreatitis: Causes and Symptoms Acute Pancreatitis Introduction Pancreatitis is inflammation of the pancreas. It is widely accepted that it is caused by pancreatic enzymes digesting their own gland. This leads to inflammation of the pancreas. There are two main forms of pancreatitis, acute and chronic. In acute pancreatitis the pancreas can usually heal itself without any lasting changes to function or structure of the gland. If the pancreas heals but then inflammation reoccurs intermittently and causes irreversible changes to structure and function then it is known as chronic pancreatitis (1). Pathophysiology The exact mechanism is not fully understood however it is believed that the initial events take place within the acinar cells of the pancreas. Injury of the acinar cells leads to an inflammatory reaction localized within the cells. If this inflammation is excessive it can lead to a systemic inflammatory response. The inflammatory process can cause systemic effects because of the presence of cytokines, such as bradykinins and phospholipase A. These cytokines may cause vasodilation, increase in vascular permeability, pain, and leukocyte accumulation in the vessel walls all leading to inflammation. Fat necrosis may also occur causing hypocalcaemia; and pancreatic B-cell injury leading to hyperglycemia (2). A marked systemic inflammatory reaction can lead to ‘distant organ damage and multiple organ dysfunction syndrome (MODS) (1). This is the primary cause of morbidity and mortality in acute pancreatitis. The disease progression can be seen in the following three steps: 1) Local inï ¬â€šammation of the pancreas, 2) A generalized inï ¬â€šammatory response, 3) Multi-organ dysfunction (1) When there is injury or disruption of the pancreatic acini pancreatic enzymes namely trypsin, chymotrypsin and elastase leak into the pancreatic tissue. These enzymes become activated and initiate autodigestion and lead to acute pancreatitis. The activated enzymes break down the pancreatic tissue and cell membranes which leads to oedema, and vascular damage which leads to hemorrhage and necrosis. Some patients who have had a severe attack of pancreatitis who survive through the initial event die following a rather minor insult that would not be life threatening normally. It is said that the two hit hypothesis comes in to play here. The initial excessive systemic inflammatory response primes the immune system so that if another event takes place (a small insult in comparison) for example a chest infection, the immune system is overwhelmed leading to an exaggerated inflammatory response which can lead to death (1). History and examination The main presentation of acute pancreatitis is epigastric pain or right upper quadrant pain radiating through to the back. In many patients sitting forward can relieve the pain a little. The patient would usually also complain of nausea, vomiting and fever. It is important to note a history of previous biliary colic and binge alcohol consumption. The patient may also be tachycardic, tachypneic, hypotensive and mildly jaundiced (2). Abdominal tenderness, distension, guarding, and rigidity are quite common as are diminished or absent bowel sounds. If the inflammation should spread to the lungs then basilar rales may be noted on auscultation of the lung. In severe cases Grey Turner or Cullens sign may also be noted (2). Aetiology There are many causes of pancreatitis. The most common causes being habitual chronic alcohol consumption and biliary stones. In western countries including the UK alcohol abuse is the most common cause of acute pancreatitis. A recent study showed that 44% of patients have alcohol as the primary risk factor for acute or chronic pancreatitis (3). Gall stones can cause pancreatitis as they may become wedged in the pancreatic duct or ampulla of Vater and obstruct the pancreatic duct, leading to release of enzymes into the parenchyma. Other less common causes include: injury (e.g. post ERCP), drugs (such as NSAIDS, azathioprine), viruses (e.g. mumps), autoimmune conditions (e.g. SLE), hyperlipidaemia, malignancy and Scorpion and snake bites (4). Investigations to be done if pancreatitis is suspected 1) Serum enzyme levels: Serum amylase in pancreatitis is more than four times the normal value and lipase is twice the normal and this is diagnostic as there is no other source other than the pancreas, but this test is not always available (5). 2) Full blood count, U+E, glucose, CRP: the CRP value is significantly lower in drug-induced acute pancreatitis and a raised bilirubin and serum aminotransferase is suggestive of gall stones. Low serum calcium levels are quite common in acute pancreatitis and hypocalcaemia is also relatively common. 3) Plain erect abdominal x-ray: this is done to exclude other causes of the symptoms such as intestinal obstruction or perforation. 4) Chest x-ray: this can show if there is a rise in one hemidiaphragm, acute respiratory distress syndrome or pleural effusions which can occur in severe cases of acute pancreatitis. 5) CT with contrast enhancement: this can be diagnostic if clinical results were inconclusive. CT may show swelling, fluid collection and change in the density of the gland. 6) Ultrasound: this is useful to see if the pancreas is swollen and if the common bile duct is dilated. It can also detect gallstones (5). Management In mild cases management is on a general medical ward. Analgesia is given to relieve the pain, usually with pethidine. Morphine is not usually used as it can have a spastic effect on the sphincter of Oddi (4). The patient is given intravenous fluids and not allowed to take anything my mouth. If the patient is vomiting severely then a nasogastic tube is considered. Oral fluids and solids can be taken once symptoms have cleared and blood tests are normal. The cause must then be treated, for example if gallstones were the cause then they must be removed. The severity of pancreatitis is determined by the Glasgow score or Ranson criteria which looks at patient demographics, electrolytes and enzyme levels on admission and 48 hours later (see 1) Glasgow prognostic score Ransons criteria * Age >55 years * WBC >15 x109/l * Urea >16mmol/l * Glucose >10mmol/l * pO2 * Albumin * Calcium * LDH >600 units/l * AST/ALT >200 units Present on admission: * Age >55 years * WBC >15 x109/l * Glucose >10mmol/l * LDH >600 units/l * SGOT >250 units/l Developing during first 48 hours: * Haematocrit fall 10% * Urea increase >8mg/dl * Serum Ca * Arterial O2 saturation * Base deficit >4meq/l * Estimated fluid sequestration >600ml In Severe cases the patent is treated in ITU. There is a high chance of multiple organ failure and infected pancreatic necrosis in these patients so if there is evidence to suggest this then intravenous antibiotics should be administered straight away. The patient should be fed via a nasogastric tube and where there are gallstones present and a high probability of a severe attack early ERCP should be done. Local Complications Pancreatic necrosis is likely if the CRP is rising and is confirmed by a CT scan. Infection occurs in 30-70% of cases of necrosis and this trebles the mortality risk. Fluid collections occurs in 30-50% of patients with acute pancreatitis but in most cases resolves spontaneously. Pancreatic abscess, acute pseudocysts and pancreatic ascites can also occur (6). Systemic complications These include pulmonary oedema, pleural effusions and ARDS with regards to the respiratory system and hypovolamenia and shock with regards to the cardiovascular system. Other complications include: disseminated intravascular coagulopathy, renal dysfunction, hypocalcaemia, hypomagnesaemia, hyperglycaemia and GI haemorrhage (6). Summary Acute pancreatitis is a disease in which there is inflammation of the pancreas. Acute abdominal pain and vomiting are the most common symptoms and increased serum concentrations of the enzymes amylase and lipase can confirm the diagnosis. Injury to the pancreas is mild in 80% of patients who recover well without complications. The rest have a more severe disease and present with local and systemic complications. Alcohol abuse and gall stones are the two most common causes of acute pancreatitis in adults and treatment of mild pancreatitis is supportive and more serious disease needs intervention from quite a few members of the multidisciplinary team (5). Improving the understanding of the pathophysiology and better investigation of the disease severity should improve the management and outcome of this compound disease (5).
